+ Before the machine beeps, before the lab values land, there's the breath I can see and hear. + This is the deep dive on the sixty-second respiratory assessment — the one habit I trust most + on a tired 2 AM shift, and the same cadence I teach when I take health screening out into the neighborhood. +

+ I run the same four moves in the same order every time, so the check is fast and nothing gets + skipped just because I'm tired. The order is deliberate: it starts with what I can see from the + doorway and ends with what only touch can tell me. +

  1. Position — before I touch anything, how is the body arranged in the bed? Upright and bracing on the arms is the tripod position; it tells me the patient is recruiting every muscle to breathe.

  3. Rate — count a full cycle for sixty seconds when the patient doesn't realize I'm watching. Adults at rest run 12–20 breaths a minute; anything sustained outside that needs a reason.

  5. Effort — is the breathing visible from across the room? Look for retractions pulling at the neck and ribs, nasal flaring, and whether the patient can finish a sentence without gasping.

  7. Auscultation — then the stethoscope: clear on both sides and top to bottom, comparing symmetry. Wheezes, crackles, and silence each point somewhere specific.
